Local police official Waheed Khan said it happened as worshippers had collected in the Kucha Risaldar mosque for Friday prayers.
Ambulances rushed the wounded to Lady Reading Hospital.
No one has claimed responsibility yet.
However, both the Islamic State group and the Pakistani Taliban organization have performed attacks of a similar nature in the past in an area that is very close to Afghanistan.
Shayan Haider, who witnessed the blast, was about to enter the mosque when the explosion threw him to a street corner.
"I opened my eyes and there was dust and bodies everywhere,'' he said.
Lady Reading Hospital Emergency department was in chaos as doctors struggled to those wounded into operating theaters.
Prime Minister Imran Khan condemned the action.
In Pakistan, Sunnis are the majority while the minority Shiites have come under attack many times.
In a latest update, the death toll rose to 63 while around over 200 were injured.
