1. Leonardo DiCaprio in Django Unchained
In Django Unchained, Leonardo DiCaprio plays a slave owner who is hosting a dinner for Django. During the dinner, DiCaprio tells him that he is aware of his plans to free his wife. He gets angry. So much so, he slams his hand on the table. He accidentally breaks a glass and slices his skin. The blood oozes out, yet he didn't stop. His hand, as can be seen in the film, is covered in blood.
2. Alex Wolff in Hereditary
In the scene in which Wolff's character Peter slams his face into a desk in the middle of the high school class. Wolff was told, it will be a soft desk. He was expecting it to be made of foam. So, he went with all his intensity and ended up dislocating his jaw and cutting his knee. The blood on the nose and other parts was real.
3. Tom Cruise in Mission: Impossible Fallout
In one of the scenes, Cruise had to jump from one building to another, but instead, he slams into the second building, pulls himself up and runs again. In this scene, Cruise's pain looked so real, right? That's because he was actually in a lot of pain. He broke his ankle, but he chose to complete the scene.
4. Brad Pitt in Se7en
In the chase scene during the rain, Pitt slams his hand through a window and breaks his tendon. The injury was so serious that the script had to be rewritten into the storyline. That is the reason why he's wearing a sling.
5. Jake Gyllenhaal in Nightcrawler
In one of the scenes, Jake has lost an important story to his competitor, and he is very angry about it. While he maintains his calm in front of everyone, he takes his frustration out on a mirror when he goes to the loo. He screams and slams it shut. He was not supposed to break it but he smashed it by accident. He sliced his thumb and got over 40 stitches later.
6. Ellen Burstyn in The Exorcist
In one of the scenes, a possessed Regan assaults her mother and slaps her. Ellen was supposed to fall back and for that, a stuntman had to pull her backward. Interestingly Ellen complained to the director that he was pulling too forcefully, but it wasn't taken seriously. In the next take that made it to the film, her scream was real as she injured her back.
7. Diane Kruger in Inglourious Basterds
Quentin Tarantino actually strangled Kruger with his own hands to make the scene authentic in the scene in the scene in which Standartenf¨¹hrer Hans "The Jew Hunter" Landa (Christoph Waltz) confronts her and leaves her breathless in anger.
8. Channing Tatum in Foxcatcher
In the role of an Olympics player, Channing Tatum aimed to be as authentic as possible; and so, he ended up smashing his head on the mirror thrice after he loses an important match. He not only shatters the mirror, he cuts his forehead as well.
9. Steve Carell in The 40-Year-Old-Virgin
Remember the chest waxing scene? It's iconic. More so, because it is real. His screams in pain are actually real. At the end of the scene, there were blood spots on the chest and tears in his eyes. He wasn't acting.
10. Sylvester Stallone in Rocky IV
To make the final fight more authentic, Sylvester Stallone asked Dolph Lundgren--who plays Captain Ivan Drago--to punch him for real. After the scene, Stallone's blood pressure shot up to 260 and he had difficulty in breathing. He had to be taken to the hospital and was admitted to ICU. His heart had slammed against his breast bone and had begun to swell.

Viggo Mortensen as Aragorn is angry at the thought that his friends might be dead. He kicks a helmet on the ground. After four takes of this shot, he broke his two toes. That scream was very, very real.
