"I was getting pushed by the bed."?Nurse Ainah Cervantes shared her account with investigators, according to a report obtained by KTVU.?"Basically, I was running backward; if I didn't run, the bed would smash me underneath."
The nurse suffered a severe laceration, which required surgical intervention.?According to documents, two embedded screws had to be extracted. Remarkably, the patient who fell from the bed to the floor escaped unharmed.
According to Fox News, despite the incident in February, the investigation didn't end until several months later.?
The report also stated that the California Department of Public Health's investigation found that the Redwood City center had "failed to provide radiologic services in a safe manner."
The investigation highlighted a series of errors leading up to the incident.?
It was noted that there were no MRI personnel present in the room when it happened. Additionally, the report indicated that the incident violated several of Kaiser's MRI safety policies.?
Furthermore, the investigation revealed that the staff had not received proper safety training and that the hospital had neglected to test the door alarm.
"The many safety failures... created a culture of unsafe practices," the California Department of Public Health's investigation stated.
Sheila Gilson, the Senior Vice President for Kaiser Permanente San Mateo, assured that the nurse promptly received the necessary care and support.
"This was a rare occurrence, but we are not satisfied until we understand why an accident occurs and implement changes to prevent it from occurring again," Gilson said.
